S.M.A.R.T.C.A.P. is our proprietary Medication Therapy Management (MTM) program designed to help our clients advocate for their own pharmaceutical care via Self Management and Reinforced Teaching, Compliance, Adherence and Persistence”.
Components of the S.M.A.R.T.C.A.P. Program include:
- Assessment of the patient’s health status
- Monitoring patient’s response to therapy
- Performing medication review
- Education and training to enhance the patient’s understanding of their own use of medications
- Special packaging for complicated medical regimens
- SmartMinder™ - a refill reminder program by phone, pager, cell phone, mail or email
- Providing coordination of benefits, information and resources to enhance patient adherence with their medication regimen.
